1. ACCEPTANCE OF TERMS
By accessing or using this website, scheduling services, or submitting payment, you agree to be bound by these Terms and Conditions. If you do not agree, you must not use this website or services.
2. SERVICES PROVIDED
One Life One Mile Consulting, Inc. provides professional preventive health screening services, workforce wellness programs, DOT and non-DOT testing coordination, consulting services, and coordinated mobile health events for transportation, corporate, and community organizations. Services are subject to availability, eligibility requirements, and applicable federal and state regulations.
3. NO MEDICAL DIAGNOSIS OR EMERGENCY SERVICES
All information provided on this website and during screening events is for informational and preventive screening purposes only. Services do not constitute medical diagnosis, treatment, or emergency care. If you are experiencing a medical emergency, call 911 immediately. Participants should consult their personal healthcare provider regarding any medical concerns.
4. VENDOR AND THIRD-PARTY SERVICES
Certain services, including laboratory testing, ultrasound services, and specialty screenings, may be provided by independent third-party vendors. One Life One Mile Consulting, Inc. is not responsible for the acts, omissions, errors, or policies of independent third parties.
5. PRICING
Pricing is determined based on the scope of services requested, number of participants, staffing requirements, vendor coordination, laboratory services, equipment needs, and event location. All pricing is quoted on a per-project or per-event basis unless otherwise agreed in writing.
6. PAYMENT TERMS
Payment terms are provided in writing prior to service delivery. Services may require deposits to secure scheduling. Corporate and event services may be invoiced under milestone-based or net 30 terms, as agreed in writing. Full payment may be required prior to service delivery for certain engagements.
7. LATE PAYMENTS
Late payments may be subject to administrative fees and interest at the maximum rate permitted by law. One Life One Mile Consulting, Inc. reserves the right to suspend or cancel services for unpaid balances.
8. CANCELLATION POLICY
Individual appointments must be canceled at least 48 hours in advance to qualify for rescheduling. Cancellations made less than 48 hours prior may result in forfeiture of fees. No-shows are non-refundable. Corporate and community events require written cancellation. Cancellations made more than 14 days prior to an event may qualify for rescheduling within 90 days. Cancellations within 14 days may result in forfeiture of deposits and staffing fees. Cancellations within 7 days are non-refundable.
9. REFUND POLICY
Due to staffing, equipment preparation, and vendor commitments, payments are considered earned once services are reserved and operational planning has begun. Individual appointment cancellations made at least 48 hours prior may qualify for a partial refund less administrative fees. Deposits for corporate and event services are non-refundable once planning has begun. Laboratory and third-party service fees are non-refundable once services have been performed or materials ordered. Approved refunds will be issued to the original payment method. Third-party processing fees, including Stripe transaction fees, may be deducted from the refund amount.
10. SERVICE INTERRUPTIONS
The company reserves the right to reschedule services due to severe weather, staffing emergencies, regulatory requirements, equipment failure, or safety concerns. Clients will be offered rescheduling options in such circumstances.
11. INTELLECTUAL PROPERTY
All website content, logos, trademarks, branding, and materials are the property of One Life One Mile Consulting, Inc. and may not be copied or reproduced without written permission. “Medical Miles™” is a trademarked branding name used by the company.
12. LIMITATION OF LIABILITY
To the fullest extent permitted by law, One Life One Mile Consulting, Inc. shall not be liable for indirect, incidental, consequential, or punitive damages arising from use of the website or services.
13. GOVERNING LAW
These Terms and Conditions shall be governed by the laws of the State of Texas.
14. CHANGES TO TERMS
We reserve the right to update these Terms and Conditions at any time. Continued use of the website constitutes acceptance of any updates.
